How to Fill Out a Health Questionnaire
As a safety measure to protect the health and well-being of employees, some organizations may requires all users to complete a health questionnaire prior to visiting the office.
In this article, we will show you how to fill out a health questionnaire in Tactic.
Health Questionnaire Reminder
Within 24 hours of a desk or conference room booking, you will receive an email notification from Tactic requesting that you take the health questionnaire prior to visiting the office.
💡 Note: The reminder period is configurable, so if you do not receive a questionnaire within 24 hours, please verify with your workspace admin what the reminder period is.
Taking the Questionnaire
Upon logging into Tactic, you will see a light yellow health questionnaire banner at the top right of the Home screen.
After clicking on the banner, you will be directed to a series of Yes or No questions. Answer them accordingly and select Submit Results.
Depending on your answers to the questions, you will either be approved for or restricted from an office visit.
If you accidentally answered any questions incorrectly, you will have two hours from submission to retake the survey.
