How to Fill Out a Health Questionnaire
As a safety measure to protect the health and well-being of employees, some organizations may require all users to complete a health questionnaire prior to visiting the office.
In this article, we will show you how to fill out a health questionnaire in Tactic.
Health Questionnaire Reminder
Within 24 hours of a desk or conference room booking, you will receive an email notification from Tactic requesting that you take the health questionnaire prior to visiting the office.
💡 Note: The reminder period is configurable, so if you do not receive a questionnaire within 24 hours, please verify with your workspace admin what the reminder period is.
Taking the Questionnaire
- Upon logging into Tactic, you will see a light yellow health questionnaire banner at the top right of the Home screen.
- After clicking on the banner, you will be directed to a series of Yes or No questions. Answer them accordingly and select Submit Results.
- Depending on your answers to the questions, you will either be approved for or restricted from an office visit.
- If you accidentally answered any questions incorrectly, you will have two hours from submission to retake the survey.
